The providers at Cache Valley Dermatology take personal care in treating all conditions of the skin, hair, and nails. They also offer cosmetic consultations and services customized to individual patients.  

Kelly W. Hubbard, MD

Board-Certified Dermatologist

Dr. Kelly Hubbard completed his Residency in Dermatology at the Mayo Clinic where he served as chief resident and is board certified by the American Board of Dermatology. Prior to this, he medical school at the University of Utah School of Medicine where he received his M.D. degree, followed by his internship at LDS Hospital in Salt Lake City.

A native of Logan, Dr. Hubbard graduated from Utah State University with a Bachelor of Music degree in Piano Performance. He then returned to Cache Valley to join the dermatology practice of Dr. Don Holdaway, who in 1975 had also come from Mayo Clinic and brought that model of excellence in patient-first dermatological care to Cache Valley. A founding member and past president of the Utah Dermatology Society, he is a Fellow of the American Academy of Dermatology, a member of the American Contact Dermatitis Society and the National Psoriasis Foundation, and serves as board member of the Mayo Clinic O-Leary Society.

Life outside of the medical clinic for Dr. Hubbard is all about family. They love to hike in the summer and ski in the winter, and learn about the world through travel whenever possible. Dr. Hubbard has sung in the American Festival Chorus for over 10 years with his wife, Dr. Nina Jorgensen. He tries to maintain his family farming roots through his only sometimes-successful garden and is occasionally spotted in the local canyons on his mountain bike.

Michael Hadley, MD

Board-Certified Dermatologist, Moh’s Surgeon

After completing his undergraduate degree at Brigham Young University, Dr. Hadley attended the University of Utah School of Medicine, graduating with his M.D., followed by a residency in dermatology at Emory University. He then completed a fellowship in Mohs Micrographic Surgery and advanced reconstructive surgery in affiliation with the University of Alabama and under the direction of Dr. Gary Monheit. After completing his training, Dr. Hadley returned to practice at the University of Utah School of Medicine and Huntsman Cancer Institute where he was an associate professor. He was involved in research, teaching and patient care.

Dr. Hadley is perhaps most proud of being selected as “Teacher of the Year” by the residents and elected as President of the Utah Dermatology Association.

Dr. Hadley currently practices as a Dermatologist at Ogden Clinic and specializes in Mohs surgery procedures and Cosmetic and Dermatologic Botox treatment. He travels to Logan once a month to offer Mohs surgery to residents of Cache Valley.




Jacob Stewart, MD

Board-Certified Dermatologist

Dr. Jacob Stewart completed his residency in dermatology at the Mayo Clinic in Rochester, Minnesota. After completing residency, he was offered a position as a consultant dermatologist in the Mayo Clinic Health System, where he practiced dermatology and was appointed Assistant Professor of Dermatology in recognition for his excellence as a clinician and academic research in the field of dermatology. He is board certified by the American Board of Dermatology.Prior to his dermatology residency, Dr Stewart completed medical school at the University of Texas Southwestern Medical School in Dallas Texas, where he received his M.D. and many scholarships and awards.

Dr. Stewart received his undergraduate education at Utah State University, where he graduated Summa Cum Laude and was awarded the USU College of Science scholar of the year award. Go Aggies!

Outside of his professional life, faith and family are of utmost importance to Dr. Stewart. He, together with his wife Cami and 4 children, enjoy hiking, biking, reading stories, and playing games together. He is always willing to play (and lose a few golf balls) at any of the local golf courses. Dr. Stewart enjoys reading, playing the guitar, unicycling, and traveling with his family whenever possible.


Jeffrey Baugh, PA-C

Physician Assistant

Jeff Baugh has practiced as a Physician Assistant for the last 10 years. His previous experience includes practicing in Emergency Medicine. Prior to joining Cache Valley Dermatology, he practiced as a Physician Assistant at West Dermatology in Northern California. As a Cache Valley native, he considers it a great privilege to “come home”. Jeff began his formal education at Utah State University where he double majored in Business and Exercise Science, graduating Magna Cum Laude and Valedictorian of his class.

He then attended Midwestern University in Glendale, Arizona where he earned his Master of Medical Science degree in the Physician Assistant Program. Hobbies include cycling, snowboarding, sports, and spending time outdoors with his wife and six children.

Kent Whitaker, PA-C, M.Ed

Physician Assistant

Kent Whitaker completed his undergraduate and graduate degrees at Weber State University in Respiratory Therapy. He worked in the NICU at McKay-Dee and Primary Children’s hospitals as well as on the LifeFlight transport team. He taught Respiratory Therapy at Weber State University for 12 years and co-authored the Comprehensive Perinatal and Pediatric Respiratory Care textbook. Kent then obtained his Physician Assistant degree at the University of Utah. He is also a Distinguished Fellow of the AAPA.

Kent is currently an Assistant Professor at Idaho State University in the Physician Assistant Program and maintains his part-time clinical dermatology practice with Dr. Hubbard. He practiced dermatology under Dr. Bradley Summers in Logan for 17 years and brought his expertise and enthusiasm to Cache Valley Dermatology after the passing of Dr. Summers in 2016. Kent loves to backpack and is a happy and devoted grandfather to 6 darling grandchildren.


Michelle Bailey, FNP-C

Family Nurse Practitioner

Michelle Bailey is a board-certified Family Nurse Practitioner with a passion for healthcare and connecting with patients. Michelle graduated as a Registered Nurse from Weber State University, then later decided to continue with her education and received a Master’s of Science in Nursing – Family Nurse Practitioner degree from Graceland University. Continuing education is an important aspect of healthcare for Michelle, either by learning for her own personal growth or by helping others through mentoring, she always strives to progress in her career.

While working as an RN, Michelle was honored to receive a peer nominated Clinical Nurse Excellence Award for her values in patient-centered care and ethics as a colleague. With over eighteen years of healthcare experience in Emergency Medicine, along with some administrative, leadership, and LifeFlight experience, she is excited to be expanding her career into Dermatology while continuing to serve the people of Cache Valley.

Life at home keeps Michelle busy, as her greatest passion is her family. She focuses on spending quality time with her husband and five children and supporting them in their interests and activities.
